Position Summary
The Physical Therapist (PT) is a licensed healthcare professional responsible for assessing, planning, and executing physical therapy services for patients in their residences, as directed by their physician. The PT collaborates with an interdisciplinary rehabilitation team to optimize patient outcomes.

Qualifications

  • Graduation from a state-approved physical therapy program accredited by the American Physical Therapy Association.
  • A minimum of six months of experience in acute care or rehabilitation settings; outpatient therapy experience may be considered if it involved medical/surgical patients.
  • Current licensure as a Physical Therapist in Pennsylvania.
  • Possession of reliable transportation for home visits, with a valid driver's license and appropriate vehicle insurance.

Key Responsibilities
Clinical Duties

  1. Conduct initial assessments for patients receiving therapy within 24 hours of referral.
  2. Perform comprehensive evaluations using diagnostic tests to assess muscle, nerve, joint, and functional abilities.
  3. Collaborate with physicians, patients, and families to develop and adjust individualized care plans based on patient progress.
  4. Educate patients and families on the physical therapy process, including home exercise programs and the safe use of assistive devices.
  5. Supervise home health aides as necessary, ensuring quality care delivery.
  6. Coordinate care for patients when therapy is the primary service requested by the physician.
  7. Plan for patient discharge in collaboration with the healthcare team.
  8. Complete required assessments and documentation in accordance with OASIS guidelines.
  9. Monitor and document patients' vital signs during visits.

Supervision of Physical Therapy Assistants
The Licensed Physical Therapist oversees the care provided by Physical Therapy Assistants (PTAs), ensuring compliance with established protocols and quality standards.

Documentation Responsibilities

  1. Submit OASIS assessments and therapy evaluations promptly.
  2. Maintain accurate and timely documentation for ongoing services.
  3. Prepare discharge summaries within specified timeframes.
  4. Ensure patient records are ready for recertification as needed.

Communication and Professional Standards

  1. Communicate effectively with physicians regarding patient evaluations and treatment plans.
  2. Provide updates to the healthcare team on patient progress and changes in care needs.
  3. Maintain patient confidentiality and adhere to professional conduct standards.
  4. Engage in continuous professional development to enhance care quality.
