Physical Therapist

Concordia Visiting Nurses is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for evaluating, planning, and implementing physical therapy services to patients in their homes or current residences as prescribed by the attending physician. You will work as a member of an interdisciplinary rehabilitation team to provide high-quality care to our patients.
Responsibilities- Evaluate patients to determine their physical therapy needs and develop individualized plans of care.
- Implement physical therapy interventions, including exercises, modalities, and education, to help patients achieve their goals.
- Communicate with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in interdisciplinary team meetings to discuss patient care and develop treatment plans.
- Graduate of a state-approved school of physical therapy accredited by the American Physical Therapy Association.
- Minimum of six months acute care or rehabilitation center experience. Outpatient therapy experience may be considered if practice included medical/surgical patients.
- Licensed as a Physical Therapist in the Commonwealth of Pennsylvania.
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state.
