Physical Therapist

About the Role:

Concordia Visiting Nurses is a Medicare-certified, state-licensed, Medicaid-approved home health agency operating in nine Western Pennsylvania counties. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion.

The Physical Therapist is a licensed professional responsible for evaluating, planning, and implementing physical therapy services to patients in their home or current residence as prescribed by the attending physician. The therapist works as a member of an interdisciplinary rehabilitation team.

Responsibilities:

• Evaluate patients and develop individualized plans of care to achieve optimal physical function and independence.
• Implement physical therapy interventions, including exercises, modalities, and education, to promote patient progress and well-being.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
• Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
• Supervise and educate Physical Therapy Assistants (PTAs) to ensure high-quality patient care.

Requirements:

• Graduate of a state-approved school of physical therapy accredited by the American Physical Therapy Association.
• Minimum of six months acute care or rehabilitation center experience. Outpatient therapy experience may be considered if practice included medical/surgical patients.
• Licensed as a Physical Therapist in the Commonwealth of Pennsylvania.
• Reliable, independent means of transportation to make home visits.
