Key Responsibilities

  • Provide independent decision making and clinical judgment to ensure safe and effective drug therapy.
  • Collaborate with healthcare professionals to develop and implement individualized patient care plans.
  • Stay up-to-date with current pharmacotherapy literature and apply knowledge to optimize patient outcomes.
  • Establish and review individual professional goals annually with the Clinical Pharmacy Manager.
  • Participate in the evaluation process to ensure alignment with hospital goals and objectives.

Requirements

  • High School diploma or equivalent required.
  • Bachelor's degree in Pharmacy required.
  • PharmD or Master's degree preferred.
  • West Virginia Board of Pharmacy Licensure required.
  • Board Certified in Pharmacotherapy or other relevant certification preferred.
  • Specialty Residency preferred.

Preferred Qualifications

  • 1-2 years of position-related experience.
  • Relevant service in applicable specialty area.
