Long-Term Care Pharmacist Job Summary:
The successful candidate will evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed. They will also maintain patient/resident databases, ensure laboratory tests are ordered and evaluated, and collaborate with the healthcare team to provide clinical and technical support.
Long-Term Care Pharmacist Responsibilities:
Maintains patient/resident databases with current and past medical history, clinical and laboratory data, and concurrent medication information
Ensures laboratory tests are ordered, evaluated, and discussed with the healthcare team
Collaborates with the healthcare team to provide clinical and technical support
Documents communication with physicians, nurses, and other healthcare team members in the medical record
Implements policies and procedures for safe drug procurement, storage, distribution, and use
Obtains and maintains required documentation in accordance with federal and state regulatory agencies and policy
Long-Term Care Pharmacist Requirements:
Bachelor's Degree required, PharmD preferred. 1+ year of pharmacist experience in Long-Term Care is preferred. Must be licensed as a pharmacist in the state of WV.
