Job Summary and Qualifications

As a Clinical Pharmacist, you will ensure the safe, effective, and economical use of medications for the patients under your care. Your advanced knowledge and professional judgment will be crucial in all aspects of your responsibilities. You will act as a patient advocate while collaborating with a multidisciplinary team of healthcare professionals.

  • Prepare and dispense medication orders as per physician requests, adhering to established protocols.
  • Verify Computerized Physician Order Entry (CPOE) orders for accuracy and safety, intervening when necessary.
  • Interpret and transcribe medication orders accurately into computerized patient profiles, following documentation practices.
  • Clarify and troubleshoot Centralized Order Entry (COE) orders.
  • Safely prepare and dispense various medications, including sterile and chemotherapy preparations, following appropriate guidelines.
  • Evaluate Total Parenteral Nutrition (TPN) for appropriateness based on established guidelines.
  • Manage controlled substances in compliance with legal requirements and corporate policies.
  • Ensure evidence-based medication therapies are safe and cost-effective.
  • Monitor medication regimens for potential interactions and appropriateness.

Required Qualifications:

  • B.S. or Pharm D. degree in Pharmacy - Required.
  • Current Pharmacist license in good standing with the State Board of Pharmacy - Required.
  • Completion of an accredited residency is preferred.
  • Two years of hospital pharmacy experience is preferred.
