Job Summary
The Qualified Intellectual Disability Professional (QIDP) will be responsible for supporting individuals with intellectual disabilities in our Day Habilitation program. The QIDP will assess individual needs, develop and implement habilitation plans, and provide supervision and training to staff.
Key Responsibilities
- Assessment and Planning
- Assess individual needs to determine habilitation requirements
- Develop and implement Staff Action Plans (SAP) and Residential Habilitation Plans (RHP)
- Monitor integration of services and make changes to SAP/RHP objectives
- Analyze data to monitor progress and write monthly progress notes
- Training and Communication
- Attend Life Plan meetings, staff meetings, and Interdisciplinary Team meetings
- Provide supervision, coaching, and training to staff
- Maintain contact with individual's family, Care Manager, and other relevant parties
- Qualifications and Work Experience
- Vaccination required
- Bachelor's Degree in human services, sociology, special education, or related field
- One year experience working with individuals with intellectual disabilities
- Knowledge of Level of Care Eligibility Determination (LCED)
- Proficiency in Microsoft Office
