We are seeking a skilled and compassionate Registered Nurse (RN) to join our Dialysis Services team at Carondelet St. Marys Hospital. As a key member of our healthcare team, you will provide direct nursing care to patients with end-stage renal disease, working collaboratively with physicians and other healthcare professionals to ensure the highest level of patient care.
Responsibilities- Implement and monitor patient care plans, ensuring that each patient receives individualized attention and care.
- Monitor, record, and communicate patient condition as appropriate, providing timely updates to the healthcare team.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care, ensuring seamless transitions and effective communication.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team, ensuring a smooth transition to post-acute care.
- Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days.
- Basic Life Support (BLS) - American Heart Association certification.
- Advanced Cardiac Life Support (ACLS) - American Heart Association, Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by individual department.
- Graduation from an accredited School of Nursing.
- Bachelors of Science in Nursing preferred.
