Job Summary

We are seeking a Certified Nursing Assistant (CNA) to join our team of compassionate caregivers. As a CNA at Moments Hospice, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Responsibilities
  • Personal Care Activities: Assist patients with daily living activities, such as bathing, dressing, grooming, toileting, and skin care.
  • Patient Care Planning: Review patient care plans and work with the interdisciplinary team to develop and implement individualized care plans.
  • Assist with Lifting and Transferring: Assist patients with lifting and transferring, ensuring their safety and comfort at all times.
  • Range of Motion and Exercise: Assist patients with range of motion, positioning, exercise, and ambulation, promoting their physical and emotional well-being.
  • Dining and Nutrition: Assist patients with dining and nutrition, ensuring their nutritional needs are met.
  • Electronic Medical Records: Maintain accurate and up-to-date electronic medical records, ensuring seamless communication with the healthcare team.
Requirements
  • Certified Nursing Assistant (CNA) License: Current CNA license in good standing.
  • 12 Months of Caregiving Experience: Minimum 12 months of experience in caregiving, preferably in a hospice or end-of-life care setting.
  • CPR Certification: Current CPR certification, preferred.
  • Hospice Experience: Previous experience in hospice care, preferred.
  • Excellent Customer Service Skills: Strong communication and interpersonal skills, with the ability to provide exceptional customer service.
  • Problem-Solving Skills: Ability to think critically and solve problems in a fast-paced environment.
  • Time Management Skills: Strong time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to Work in an Electronic Medical Record System: Proficiency in using electronic medical record systems, with the ability to navigate and update records accurately.
  • Ability to Lift 50 lbs.: Ability to lift 50 lbs., with or without reasonable accommodation.
  • Good Understanding of Body Mechanics: Strong understanding of body mechanics, with the ability to promote patient safety and comfort.
  • Valid Driver's License: Valid driver's license, required.
