About the Role

We are seeking a highly motivated and compassionate Care Extender to join our team at the Tammy Lynn Center (TLC). As a Care Extender, you will play a vital role in supporting our members by providing information and assistance with accessing services and community resources.

Key Responsibilities
  • Collaborate with Care Managers to conduct initial and ongoing outreach and engagement activities with members, supporting them in meeting their health-related needs.
  • Link members to social services and community resources, including housing, food, employment, and other essential services.
  • Track referrals for community-based resources and social support services to ensure needs are being met.
  • Assist members in scheduling appointments, arranging transportation, and following up with them to ensure they receive the support they need.
  • Participate in care team meetings and planning activities to identify members' needs and develop strategies to address them.
  • Develop educational materials and resources to help members understand health promotion, self-advocacy, and navigating service systems.
  • Support member engagement in health promotion activities and educate them about mental or physical health, abuse, and available community resources.
  • Encourage members to develop work skills and participate in social, recreational, or therapeutic activities that enhance interpersonal skills and develop social relationships.
  • Maintain accurate documentation of contacts and activity within the care management system.
Requirements
  • Lived experience with intellectual or developmental disabilities (I/DD), traumatic brain injury (TBI), or a behavioral health condition, with a demonstrated knowledge of and direct personal experience navigating the NC Medicaid service delivery system.
  • At least two years of paid experience performing Care Extender functions, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
