Job Summary

The Care Coordinator RN is a registered nurse responsible for facilitating the physician care plan for patients during their hospital stay. This role involves collaborating with interdisciplinary teams to confirm and coordinate clinical care, ensuring optimal patient outcomes and resource utilization.

Key Responsibilities

  • Collaborate with physician offices, physician advisors, case managers, and unit-based case managers to assess patient needs and develop comprehensive care plans.
  • Act as a patient advocate, ensuring compliance with federal, state, and third-party utilization management regulatory requirements.
  • Work closely with healthcare teams to anticipate and integrate clinical care needs, optimizing patient outcomes and resource utilization.

Requirements

  • 3 years of experience in nursing or a related field, with a focus on case management or utilization management.
  • Graduate nursing degree required; Bachelor's degree in nursing preferred.
  • Registered nurse licensure required; Certified Case Manager or Commission for Case Management Certification preferred.

Work Schedule

Monday to Thursday, 9:00 am to 6:00 pm, with no weekend or call requirements.
