Position Summary: Functioning as the patient's advocate, the Care Coordinator is responsible for the non-clinical functions of the Care Team. The Care Coordinator works in conjunction with the physician and the medical assistant (MA) to provide an efficient and high-quality care experience for the patient. Travel to various AUNC locations with a Care Team may be required occasionally based on staffing needs (vacation coverage, unplanned absences, etc).
Essential Job Responsibilities:
- Schedule all office appointments & surgeries for patients.
- Obtain and input patient demographics and basic payer information into computer system; update information as necessary when booking appointments.
- Obtain referrals & authorizations.
- Assist in scanning into Urochart patient paperwork, including new patient forms, lab, CT, X-ray and pathology results, correspondence, outside records and other documents as necessary. Collaborates with medical records staff as needed.
- Handle all telephone calls for physician and nurse or MA; take messages as necessary, and prioritize urgent calls appropriately.
- Prepare patient charts for visits and surgeries ahead of time.
- Act as liaison for physician's primary referrers.
- Manage physician's outgoing referrals.
- Educate patients about Care Team; helps to set patient expectations regarding who to call and how calls/concerns will be managed.
Education Requirements: High School diploma.Experience Required: Minimum one year of experience in surgery scheduling in a medical practice or hospital setting. Experience scheduling patient appointments and obtaining authorizations via practice management software, required.
Performance Requirements (Knowledge, Skills & Abilities):
Knowledge:
- the medical practice protocols related to scheduling patient appointments and scheduling surgery,
customer service principles and techniques.
Skills in:
communicating effectively with physicians/clinicians about scheduling preferences,
maintaining master appointment schedule on computer, and
communicating effectively with patients, making them feel comfortable and welcome within the practice.
Ability to:
multi-task effectively, dealing pleasantly and efficiently with multiple phone calls (cell & land line as required), in-office patients, staff, physicians and others;
troubleshoot situations and respond appropriately;
communicate calmly and clearly with patients about their appointments or surgeries in all circumstances, including when they are ill or have an emergency; and
work not only as an effective member of their individual Care Team but as a member of the group of Care Coordinators within the practice.
Equipment Operated: Standard office equipment including computers/internet, fax machines, copiers, printers, scanners, telephones, etc.
Work Environment: Position is in a well-lighted, temperature-controlled office environment.
Mental/Physical Requirements: Daily activity is 80 percent sitting and 20 percent walking or standing. Requires corrected vision and hearing to normal range.
