**Job Summary:**

The Ambulatory Care Nurse II is a registered nurse responsible for providing clinical expertise and leadership in the management of patient care services. This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans to maximize the ability to remain in a community setting.

**Key Responsibilities:**

  • Collaborate with physicians to identify at-risk patient populations who require care management services.
  • Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team.
  • Support the mission, values, and management of patient care services.
  • Actively support and incorporate ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, and WakeMed's Division of Nursing Professional Practice model.

**Requirements:**

  • 2 years of nursing experience or no experience required with preferred education.
  • Diploma in nursing or associate's degree in nursing required; bachelor's degree in nursing preferred.
  • Registered nurse licensure required.

**Work Schedule:**

8-5 hours of work per day, with no weekend requirements and no call requirements.
