**Job Summary**
The Ambulatory Care Nurse II is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role requires a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.
**Key Responsibilities**
- Identify at-risk patient populations who require care management services, in collaboration with physicians.
- Serve as a patient/family advocate, acting as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
- Support the mission, values, and management of patient care services, while actively promoting the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
**Requirements**
- 2 years of nursing-related experience or no experience required with preferred education.
- Diploma in nursing or Associate's degree in nursing required; Bachelor's degree in nursing preferred.
- Registered Nurse licensure required.
**Work Schedule**
8:00 AM - 4:30 PM, with occasional weekend and call requirements as needed.
