Case Manager

At the Center for Addiction Treatment, we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing case management, psychoeducation, and clinical documentation to our patients.
Key Responsibilities:- Aid patients in obtaining necessary services, such as housing assistance, food, clothing, medical services, educational services, vocational services, etc.
- Develop and implement individualized treatment plans, and other clinical documentation.
- Build and maintain positive relationships with referral agencies.
- Able to maintain a minimum of 50% productivity requirements.
- Facilitate psychoeducation groups using provided material.
- Certified by the Ohio Chemical Dependency Professionals Board.
- Previous experience is not required but prefer one-year paid experience in case management.
- Basic computer and Microsoft office skills are required.
- Reporting: Reports unusual incidents through variance reporting to agency management.
- Infection Control: Maintains infection control standards by practicing universal precautions.
- HIPAA: Knowledge and ability to apply HIPAA laws and Federal Regulations protecting patient privacy and rights.
- Patient Rights: Adheres to all patient rights standards as well as serves as a resource to patients regarding those rights.
- Professional: Exhibits professional behaviors such as maintaining the CAT Code of Ethics, working cooperatively with others following CAT personnel polices etc.
- Adaptability: Adapts to change, open to new ideas and responsibilities.

Reports to the Residential Supervisor. Supervision occurs through regular individual supervisory sessions, participation in group supervision meetings, daily census report, and review of compliance with clinical performance standards and measures through chart audits.
