- Initiate, coordinate, and monitor evaluations to identify client needs in areas such as living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, vocational/educational, social supports, leisure, and recreation.
- Act as an advocate for clients and coordinate programs of support throughout the community to benefit individuals with mental illness.
- Provide crisis intervention services as needed to reduce symptoms.
- Accompany clients to appointments, community events, and outings.
- Work with clients to reduce behavioral health symptoms.
- Monitor school and home environments when necessary.
- Complete required documentation capturing client activities, goals, and progress.
- Recruit clients and staff.
- Manage intake processes.
Requirements:
- High School diploma required (verified)
- Associate degree in social services or related field preferred
- 2-3 years of related mental health, social service, or working with youth experience
- Driver's License, Insurance, and car required
- Must pass background check.
