Job Title: Behavioral Health Therapist

At InnovAge, we are seeking a highly skilled Behavioral Health Therapist to join our team. As a Behavioral Health Therapist, you will play a critical role in developing and delivering personalized care plans for our participants.

Responsibilities:
  • Conduct initial assessments and provide clinical interventions to participants and their families.
  • Develop and implement treatment plans that address the unique needs of each participant.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to participants and their caregivers on mental health topics and strategies for managing behavioral complexities.
  • Participate in IDT meetings, conferences, and committees to ensure continuity of care and effective communication.
Requirements:
  • Master's degree in behavioral health, social work, or psychology.
  • 1+ year of experience working in an integrated care setting.
  • Licensed Mental Health Professional (LCSW, LMFT, LPC, or PsyD/PhD).
  • CPR certification or ability to obtain within 30 days of employment.
Preferred Qualifications:
  • Knowledge of geropsychiatric populations and experience working with older adults.
  • Training or experience in group and family interventions.
  • Experience with conflict management, mediation, de-escalation, and crisis mitigation.
  • Knowledge of chronic disease management and its psycho-social implications.
