Job Summary

The Behavioral Health Specialist will work in coordination with primary care providers to deliver integrated care services, providing brief therapy behavioral health interventions to SAC Health medical clinic patients.

Key Responsibilities
  • Conduct assessments and screenings, and provide therapeutic interventions as needed.
  • Develop treatment plans that are integrated with the patient's medical treatment plan.
  • Work closely with the medical team to meet the needs of individual patients.
  • Coordinate case management activities and supervise students who rotate through SAC Health Behavioral Health.
Requirements
  • Master's Degree in Social Work (MSW) required.
  • Eligible for LCSW licensure or PhD of PsyD within three (3) years of date of hire.
  • Minimum of 1-2 years of licensed clinical experience preferred.
Essential Skills
  • Ability to use behavioral health intervention as part of an integrated care response.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary environment.
