Unison Behavioral Health Group is seeking a highly skilled Care Coordinator Supervisor to lead a team of care coordinators in the Ohio Rise Program. This role is responsible for promoting the recovery of children and families participating in the program through the supervision of direct service care coordinators.
Key Responsibilities:- Implement the required model of care and provide individualized care based on client need.
- Ensure assigned staff participate in ongoing training and coaching to ensure consistency in delivering care coordination.
- Monitor completion of paperwork by assigned staff, including initial assessments, CANS assessments, care plans, and discharges.
- Bachelor's degree in social work, psychology, or a related field with a master's degree preferred.
- LSW, LPC, LISW, or LPCC preferred.
- Minimum three years' experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
- Must be proficient in the use of computers and software.
- Possess a valid driver's license, reliable transportation, and be able to be insured through the agency commercial policy and have personal auto insurance.
- Prior supervisory experience preferred.
- Hybrid work model includes working from home and in the community.
