**Job Summary**

Community Empowerment Resources is seeking a skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will work with individuals diagnosed with serious and persistent mental illness, providing clinically based treatment relationships and helping them integrate into the community using a structured recovery plan.

**Key Responsibilities**

  • Develop and implement individualized treatment plans
  • Provide case management services, including obtaining psychiatrist visits, medications, housing, substance abuse treatment, and legal coordination
  • Work with the treatment team, communicate with stakeholders in the community, and complete vital documentation
  • Host skill-building and recreational groups onsite to aid in client recovery

**Requirements**

  • High School Diploma/GED or equivalent
  • Preferred Bachelor's degree in Psychology, Social Work, Health, Nursing, Counseling, or similar degree with coursework in psychology
  • Preferred 6-12 months supervised experience with severely mentally ill populations

**Benefits**

  • $18 - $20 per hour, depending on background, education, and performance

**Work Environment**

The facility operates 24/7/365, with no on-call required. This is an indoor/outdoor therapeutic environment with ramps, stairs, and gravel/uneven ground.

**Shift Options**

  • Full-time: Four or five 8-hour shifts (32-40 hours per week)
  • Part-time: Two or three 8-hour shifts (16-24 hours per week)
