Certifications:
  • Homecare Home Base and OASIS

Must-Have:
  • COVID and flu vaccines required (declinations not accepted)
  • Bachelor's degree preferred
  • Case management experience preferred

Job Description:
Key Responsibilities:
  • Provide excellent patient care through collaboration, development, implementation, revision, and reporting of the case management program.
  • Act as a liaison between the patient, family, healthcare providers, and healthcare benefactors to optimize outcomes.
  • Serve as a consultant to the healthcare team on specific patient items.
  • Lead coordination of patient care with other disciplines within the care team, monitoring appropriateness and timeliness of care.
  • Ensure the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, procedures, and other aspects of patient care appropriate for acute care.
  • Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans.
  • Focus on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients.
  • Collaborate with acute care case managers to ensure appropriateness of ongoing care.
  • Coordinate transfer of patients to tertiary centers, including transferring patient information required for continuity of ongoing treatment and services.
  • Provide oversight and collect data required for regulatory and accreditation compliance.
  • Manage frequent emergency department visitors by conducting focus study reviews of previous and current admissions.
  • Participate in rotating weekends as needed (2-3 per month).
