Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Oak Street Health. As a Nurse Practitioner, you will play a vital role in providing exceptional primary care to our patients, working collaboratively with our physicians and care team to deliver high-quality, patient-centered care.

Key Responsibilities:
  • Provide routine and acute care to patients, including conducting office visits, administering wellness visits, and health risk assessments.
  • Care coordination with other providers, specialists, testing facilities, and agencies to ensure seamless patient care.
  • Population health leadership, working with the care team to develop and implement evidence-based care plans.
  • Educate patients on their health conditions, care plans, and treatments, promoting health literacy and patient empowerment.
  • Participate in Oak Street Health promotional activities, representing our organization and values in the community.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license and DEA license.
  • US Work Authorization.
  • Someone who embodies being 'Oaky', radiating positive energy, assuming good intentions, creating an unmatched patient experience, driving clinical excellence, taking ownership, and delivering results.
Preferred Qualifications:
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Passion for teamwork and collaboration, with a desire to be part of an innovative model focused on empirically-guided population health.
  • Bilingual proficiency in applicable areas.
