Job Summary
The Support Coordinator/Service Coordinator/Case Manager plays a vital role in our team, responsible for providing case management activities to meet the unique needs of our clients. This position involves developing individualized care management plans, implementing services, and monitoring clinical outcomes to ensure professional, comprehensive, and cost-effective care. The ideal candidate will assist clients in identifying and communicating their needs, preferences, and requests for services and supports, including housing, medical, social, vocational, and other community services.

Key Responsibilities

  • Complete assessments to identify client needs and desires for service delivery.
  • Develop and modify Person-Centered Service Plans annually or as needed.
  • Ensure client choice of providers by providing information for informed decision-making.
  • Coordinate with other individuals and entities essential to client care delivery.
  • Research and develop alternative solutions to meet client needs.
  • Ensure confidentiality and maintain private health information.
  • Attend required trainings and recertification classes.
  • Confer with supervisor/director to understand client needs and abilities.
  • 50% travel to clients as needed.

Qualifications
  • BS/BA degree in related coursework.
  • Minimum 3 years of experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
  • Experience with Microsoft Office and state-maintained databases.
  • Maintain proficiency in company-sponsored training and certifications.
  • CPR/First Aid certification and updated state-related clearances.
  • Valid Driver's License, including personal vehicle insurance coverage.
