Admission and Discharge Clinical Nurse Specialist

As an Admission and Discharge Registered Nurse at Henrico Doctors' Hospital Forest, you will play a critical role in ensuring the delivery of high-quality patient care. Your primary responsibility will be to assess patients for needs and understanding of their care after discharge, working closely with the care team to ensure a smooth transition of care.
Key Responsibilities- Assist with throughput by facilitating timely admissions and discharges
- Serve as a liaison between the patient and care team, assisting with coordinating discharge care and initiating patient teaching, admission education, and discharge planning
- Collaborate with inpatient directors to positively impact the patient's overall experience
- Nursing graduate with RN Diploma or Associate of Science in Nursing required, BSN preferred
- Minimum of 2 years acute care nursing experience
- BLS certification
- Licensed Registered Nurse in the Commonwealth of Virginia or has a Compact license
The estimated salary for this role is $85,000 - $110,000 per year, depending on experience.
