">Job Description
">As a Licensed Practical Nurse (LPN) at Sedgewood Commons, you will be responsible for providing efficient and effective nursing care to patients while collaborating with a multidisciplinary team to ensure optimal outcomes. Your duties will include:

- Evaluating patient conditions to collect data, observe condition, and report changes in condition">
- Contribute to nursing assessments and care planning">
- Supervise and monitor patient care provided by unlicensed staff">
- Administer medications and performs treatments per physician orders">
Required Skills & Qualifications
">To succeed in this role, you must:

- Be a graduate of an approved school for practical or vocational nursing">
- Hold current licensure by the State Board of Nursing or pending licensed practical nurse licensure">
- Possess experience in rehabilitative or geriatric nursing, although it is not mandatory">
- Maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility">
