About the Job

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Glens Falls Center for Rehabilitation and Nursing. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our facility.

Key Responsibilities
  • Admission and Assessment: Collect and record vital information from new residents, including health details and medication administration.
  • Medication Management: Administer medications and injections as needed, ensuring accurate dosing and documentation.
  • Wound Care: Treat and dress wounds and bedsores, promoting optimal healing and patient comfort.
  • Supervision and Support: Supervise Certified Nursing Assistants (CNAs) and provide guidance and support as needed.
  • Personal Care: Assist residents with daily living activities, including dressing and personal hygiene.
  • Monitoring and Documentation: Monitor residents' food and liquid intake and output, maintaining accurate records and reports.
Requirements
  • Teamwork and Collaboration: Demonstrate excellent communication and teamwork skills, working effectively with colleagues to achieve patient-centered care.
  • LPN Licensure: Possess a valid LPN state license and be in good standing with the State Registry.
