As a Clinical Nurse 2 Colleague on our Home Hospice Resource Team, you will work collaboratively with a multidisciplinary team to provide holistic care to patients and their families.

Our team takes a proactive approach to pain and symptom management, addressing the physical, emotional, and spiritual needs of our patients.

As a key member of our team, you will be responsible for providing high-quality patient care, working closely with patients, families, and healthcare professionals to develop and implement individualized care plans.

Key responsibilities include:

  • Providing direct patient care through the application of the nursing process
  • Collaborating with the interprofessional healthcare team to facilitate coordination of care across the continuum
  • Expanding knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations
  • Seeking feedback in assuming leadership roles and providing feedback for improved clinical practice
