Job Summary

We are seeking a highly skilled and compassionate Staff Coordinator CNA Required to join our team at The Resource Exchange Inc. As a key member of our staff, you will play a vital role in supporting individuals with long-term care needs and their families.

Key Responsibilities
  • Conduct service plan development tasks and duties in and around the community, including meeting with individuals in private homes, group homes, and community locations.
  • Partner with families to identify needs, develop resources, and complete individual family support plans.
  • Complete eligibility determination for children at age 5 and present information to the family regarding public school transition process.
  • Assist with facilitating the Human Rights Committee and completing reviews of Incident Reports for individuals served by TRE.
  • Participate in professional goal creation through the development and implementation of your career aspirations at TRE.
Requirements
  • Bachelor's degree in a human behavioral science or related field of study.
  • Experience working with long-term services and supports (LTSS) population, in a private or public agency, or a combination of LTSS experience and education.
  • Knowledge of community services and resource development.
  • Ability to possess cultural awareness, emotional intelligence, and be aware of unconscious biases.
