The Licensed Practical Nurse (LPN) Clinical Care Leader (CCL) - Long-Term Care (LTC) is a key role within our organization, responsible for delivering high-quality patient care in a long-term care setting. As a Clinical Care Leader, you will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our residents.
Key Responsibilities- Utilize the nursing process to assess, diagnose, plan, implement, and evaluate patient care
- Collaborate with the healthcare team to develop and implement care plans
- Document patient history, symptoms, medications, and care provided in compliance with applicable standards and regulations
- Utilize technology to enhance patient care and collaborate with the healthcare team
- Delegate tasks to and supervise unlicensed personnel as needed
- Assume overall nursing accountability for a patient assignment and serve in leadership roles on departmental task forces and committees
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing is preferred
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possess multi-state licensure privileges
- Obtains and maintains required department-specific competencies and certifications
