About the Role:

The RN Care Manager is responsible for coordinating multiple disciplines and resources to facilitate the transition of patients to the appropriate level of care. This role requires the development of a plan of care to ensure the highest quality care with the best outcomes for patients in the most cost-effective manner.

Key Responsibilities:

Utilize the nursing process, evidenced-based practice, and unit-specific competencies to complete an accurate and timely assessment of patients' needs, goals, preferences, and resources available.
Facilitate the development of a plan of care in collaboration with patients, families, caregivers, and healthcare teams. Determine the appropriate level of care and coordinate interdisciplinary treatment and plans of care.
Arrange access to care transition needs, primary care providers, physician specialists, and similar services/equipment as necessary.
Evaluate progress toward goals/expected outcomes of the plan, facilitate the progression of care, and ensure revisions in response to changes in patients' needs and conditions.
Proactively identify, communicate, and resolve barriers that impede the progression of care.
Communicate information effectively to healthcare teams, patients, caregivers/families, and those involved in treatment and patients' care.
Advocate on behalf of patients/families and caregivers for service access and insurance benefits allowing informed choice within the constraints of regulations, and for the protection of patients' health, safety, and rights.
Evaluate options available and balance cost and quality to ensure optimal clinical and financial outcomes.
Collaborate with multiple disciplines internally and externally to effectively and efficiently identify and provide available community resources/potential partners, and advocate for resolution of gaps in available resources.
Collaborate with managers, physicians, medical directors, advisory groups, and treatment teams for issues related to physician practices and best practices for patients' plans of care.

Qualifications:

Bachelor's of Science in Nursing (BSN)
Current Wisconsin RN License
American Heart Association Healthcare Provider Basic Life Support (BLS)

Work Environment:

Climate-controlled office setting with daily movement throughout the facility. Interaction with department members and other healthcare providers.

This role requires compliance with department-specific competencies. 1.0 FTE, 12-hour shifts with every 5th weekend on rotation. Moves between Neenah and Appleton locations.
