About the Role

The RN Care Manager is a key member of our healthcare team, responsible for coordinating multiple disciplines and resources to facilitate the transition of patients to the appropriate level of care.

This role requires a strong nursing background, with the ability to develop and implement plans of care that ensure the highest quality care with the best outcomes for patients in the most cost-effective manner.

Key Responsibilities
  • Utilize the nursing process, evidenced-based practice, and unit-specific competencies to complete accurate and timely assessments of patient needs, goals, preferences, and resources available.
  • Facilitate the development of plans of care in collaboration with patients, families, caregivers, and healthcare teams, determining the appropriate level of care and coordinating interdisciplinary treatment and plans of care.
  • Arrange access to care transition needs, primary care providers, physician specialists, and similar services/equipment as necessary.
  • Evaluate progress toward goals/expected outcomes of plans, facilitate the progression of care, and ensure revisions in response to changes in patient needs and condition.
  • Proactively identify, communicate, and resolve barriers that impede the progression of care.
  • Communicate information effectively to healthcare teams, patients, caregivers/families, and those involved in treatment and patient care.
  • Advocate on behalf of patients/families and caregivers for service access and insurance benefits, allowing informed choice within the constraints of regulations, and for the protection of patients' health, safety, and rights.
  • Evaluate options available and balance cost and quality to ensure optimal clinical and financial outcomes.
  • Collaborate with multiple disciplines internally and externally to effectively and efficiently identify and provide available community resources/potential partners, and advocate for resolution of gaps in available resources.
Requirements
  • Bachelor's of Science in Nursing (BSN)
  • Current Wisconsin RN License
  • American Heart Association Healthcare Provider Basic Life Support (BLS)
Work Environment

This role is based in a climate-controlled office setting with daily movement throughout the facility, interacting with department members and other healthcare providers.

Position requires compliance with department-specific competencies.

1.0 FTE, 12-hour shifts with every 5th weekend on rotation.
