About the Role

The RN Care Manager is a key member of our healthcare team, responsible for coordinating the transition of patients to the appropriate level of care. This role requires a strong understanding of the patient's needs, goals, and preferences, as well as the ability to develop and implement a plan of care that ensures the highest quality outcomes in a cost-effective manner.

Key Responsibilities
  • Assessment and Planning: Utilize the nursing process and evidence-based practice to conduct a thorough assessment of the patient's needs, goals, and preferences. Develop a plan of care that takes into account the patient's individual circumstances and ensures the best possible outcomes.
  • Care Coordination: Facilitate the development of a comprehensive care plan that involves the patient, family, caregivers, and healthcare team. Coordinate the transition of care to the appropriate level, ensuring seamless communication and collaboration among all stakeholders.
  • Resource Allocation: Identify and allocate resources to support the patient's care plan, including access to primary care providers, physician specialists, and other necessary services.
  • Progress Monitoring: Evaluate the patient's progress toward their goals and expected outcomes, making adjustments to the care plan as needed to ensure optimal results.
  • Communication and Advocacy: Communicate effectively with the healthcare team, patient, caregivers, and family members to ensure that all parties are informed and engaged in the care process. Advocate on behalf of the patient to ensure access to necessary services and benefits.
Requirements
  • Education: Bachelor's of Science in Nursing (BSN)
  • Licensure: Current Wisconsin RN License
  • Certifications: American Heart Association Healthcare Provider Basic Life Support (BLS)
Work Environment

This role is based in a climate-controlled office setting with daily movement throughout the facility. The RN Care Manager will interact with department members and other healthcare providers to ensure seamless communication and collaboration.
