Nurse Care Specialist

Job Summary:
Veracity Resourcing and Services is seeking a skilled LPN/RN to provide high-quality patient care. The successful candidate will work under the direction of the Nurse Manager, utilizing their entire scope of practice to deliver competent care and safety to residents.
Key Responsibilities:
- Conduct patient assessments and complete necessary documentation to prepare them for providers.
- Identify chronic conditions and preventive health maintenance issues to be addressed during office visits.
- Efficiently manage process issues and office flow to ensure smooth operations.
- Provide patient education to improve medication compliance and management of chronic disease, enhancing health literacy.
- Advocate for patients to reduce barriers to care within and outside the facility, connecting them to necessary resources.
- Lead the clinical team and utilize critical thinking to problem-solve day-to-day issues in the clinic.
- Provide oversight to patient care and appointment flow in the clinic site.
Requirements:
- Graduation from an accredited school of nursing required.
- Registered Nurse License in the state.
- CPR Certification.
Pay and Benefits:
- LPN Pay Rate: $32/hr.
- RN Pay Rate: $43/hr.
