About the Role

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Home Health Care Nurse. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Implement and manage individualized care plans, in collaboration with our interdisciplinary team, to ensure optimal patient outcomes.
  • Conduct thorough patient assessments, identify care needs, and develop effective care plans to address those needs.
  • Administer medications, treatments, and therapies as prescribed by physicians, and document all care activities in our electronic medical record system.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with our care team to ensure seamless transitions of care and optimal patient outcomes.
Requirements
  • Graduate of an accredited school of nursing with a current LPN/LVN license.
  • At least six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience preferred.
  • Ability to speak, write, read, and understand English.
  • Must be able to travel and have visual and hearing acuity.
