Mass General Brigham is seeking a skilled Home Health Charge Nurse (RN) to provide high-quality patient-centered care in the home setting.
This is a 36-hour per week, day shift position that will service NWH, BWH, and BWFH.
Key Responsibilities- Develop and implement individualized care plans that meet the unique needs of patients.
- Provide patient-centered care in the home setting.
- Collaborate with the interdisciplinary team to ensure seamless care transitions.
- Communicate effectively with patients, families, and healthcare providers.
- Manage and prioritize multiple responsibilities in a fast-paced environment.
- Current Massachusetts license as a Registered Nurse.
- Minimum of one year of recent nursing experience in an acute or emergency setting.
- Home care experience is a plus.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
