Your Role
As a Nurse Practitioner, you'll conduct in-home visits in the San Francisco Bay Area, providing high-quality, evidence-based patient care within the primary care scope of practice. You'll work with moderate to complex patient cases under minimal supervision from other physicians. We're seeking skilled professionals trained in comprehensive first contact and continuing care for individuals with diagnosed and undiagnosed signs, symptoms, or health concerns.
About Your Work
As a Nurse Practitioner, you'll assess and evaluate patient health needs, providing healthcare services that require advanced knowledge and skills. You'll utilize patient-centered care concepts to manage acute and chronic conditions, as well as preventative care, within the full scope of clinical practice. Your responsibilities will include:
- Performing comprehensive health assessments of patients in person and via telehealth
- Utilizing excellent communication skills to ensure continuity of care for patients, including regular communication and co-management of care with Primary Care Providers, Specialists, and Allied Health Providers
- Identifying actual or potential health risks based on observation of physiological and/or behavioral data and responding appropriately
- Formulating concise problem lists based on clinical findings
- Formulating a complete, concise, and clear plan, including follow-up care
- Providing and evaluating patient care according to protocols and quality assurance program criteria
- Submitting and following up on timely referrals for needed preventative care, including labs and radiology, and addressing all aspects of a comprehensive evaluation, including but not limited to, Medication Review, Reconciliation Post Discharge, Transition of Care, Care for Older Adults, Meaningful Use, Behavioral Health, Social Determinants of Health, Advance Care Planning, and Patient Experience
- Capturing HEDIS Quality Gaps during appointments, including Diabetes, Cancer Screenings, Nephropathy, and Statins
- Collaborating with HCC Coders on clinical documentation review before submission of chart for claim and diagnoses accuracy
- Maintaining a general knowledge of diagnosis codes and supporting documentation required
- Active and good standing RN licensure and Nurse Practitioner Certification in the State of California
- Active and good standing NPI number
- Post-graduate training in area of responsibility
- 2-4 years as a Clinical/Family Nurse Practitioner
- For Nurse Practitioners, possession of a current Furnishing Number issued by the California Board of Registered Nursing AND Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) OR Certification by the National Certification Corporation as a Women's Health Care Nurse Practitioner (WHNP)
