Job Summary

In this role, you will provide high-quality nursing care to clients in their homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities
  • Provide skilled nursing visits to clients in their homes, as directed by the RN Case Manager.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Monitor and report changes in client condition to the Case Manager and physician, as necessary.
  • Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in the development, implementation, and evaluation of the Agency Quality Improvement Program.
Requirements
  • Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
  • Current CPR certification.
  • Valid driver's license and appropriate automobile insurance coverage.
  • Minimum of one year of experience in an acute care setting or equivalent.
