Role Overview:

The Clinical Coordinator at the AH Transplant Institute (AHTI) is an integral member of a multidisciplinary team dedicated to providing personalized care to patients pursuing or having undergone organ transplantation. Key responsibilities encompass managing patient care throughout all stages of transplantation, including pre-operative, peri-operative, and post-operative phases. This role emphasizes outstanding customer service and fosters respectful relationships with all stakeholders.

  • Participates in a 24-hour on-call rotation
  • Responsibilities and core competencies may differ by organ group
  • Pre-Operative:
    • Facilitates and coordinates tailored evaluations for potential organ recipients or living donors, including gathering medical history, educating patients and families, identifying co-morbidities, and collaborating with transplant team members.
    • Communicates effectively with the multidisciplinary transplant team and referring physicians.
    • Maintains accurate nursing documentation throughout the evaluation process.
    • Ensures compliance with Organ Procurement Transplant Network and United Network for Organ Sharing policies.
    • Coordinates care for candidates awaiting transplant or living donation.
  • Peri-Operative:
    • Coordinates the logistics of potential transplant surgeries.
    • Facilitates communication among patients, transplant physicians, surgical teams, and hospital departments to ensure smooth patient admissions.
    • Maintains thorough documentation of transplant surgeries in accordance with relevant policies.
  • Post-Operative:
    • Coordinates outpatient care to promote optimal recovery, reinforcing patient education and monitoring for potential complications.
    • Assesses patient adherence to post-transplant care plans and medication regimens.
  • Bone Marrow Transplant:
    • Facilitates patient entry into the program by coordinating appointments for necessary tests and services.
    • Expedites all BMT protocols and informed consent processes.
    • Collaborates with the multidisciplinary team and healthcare personnel regarding patient status.
    • Schedules pre- and post-bone marrow transplant evaluations in conjunction with attending physicians.
    • Coordinates logistics for stem cell collections and communicates with relevant departments.
    • Facilitates financial authorizations for BMT care through communication with insurance providers.
    • Educates patients and families on all aspects of BMT care.
    • Assists in protocol updates and data collection for program reporting.
    • Provides clinical support and training for new staff members.

Qualifications:

Required Licensure and Certifications:

  • Current valid State of Florida or multi-state license as a Registered Nurse
  • BLS (Basic Life Support) certification
  • ACLS (Advanced Cardiac Life Support) certification if working in the BMT unit
