We are looking for a highly skilled and compassionate individual to join our team as a Care Coordination Specialist. In this role, you will be responsible for coordinating patient care, providing education, and supporting patients and their families through challenging times.

The ideal candidate will have a strong background in social work and experience working in a healthcare setting. You will be expected to maintain accurate records, communicate effectively with interdisciplinary teams, and provide high-quality patient care.

Estimated Salary: $60,000 - $80,000 per year

Requirements:

  • 4-year Bachelor's Degree in social work or related field
  • Previous experience in healthcare preferred
