Job Description- Care Coordination: Work closely with residents, family members, and healthcare professionals to develop and implement personalized care plans.
- Medication Management: Assist with medication administration, monitoring resident health, and reporting any concerns to supervisors.
- Supportive Services: Provide emotional support, guidance, and education to residents and their families, promoting independence and quality of life.
**Requirements:**
- Licenses and Certifications: Current LPN license in applicable state and CPR certification.
