The Program and Outreach Director will report directly to the Executive Director.

OBJECTIVE

Resident Engagement Coordinator

The Program and Outreach Director is tasked with designing, leading, and managing a resident-focused activities and outreach initiative that embodies the Phoenix Senior Living philosophy, integrating wellness into every facet of the lives of the seniors we serve. This role ensures that all dimensions of wellness—emotional, purposeful, physical, social, intellectual, and spiritual—are woven into the services we offer.

RESIDENT SUPPORT

Resident Engagement Coordinator

Engages in the development of Personalized Service Plans (PSP).

Consults the Resident Profile and its addendum for each new resident.

Ensures a diverse range of suitable and personalized activities that encompass all dimensions of wellness, available throughout the day and evening, with participation from resident care associates.

Maintains the PSL Activity & Program Guidebook, tracking daily resident interests and participation in activities.

Utilizes demographic profile information to organize small groups or clubs that cater to similar needs and preferences.

Recognizes changes in resident conditions and takes appropriate action.

Assists residents in moving to and from activity rooms as needed.

In memory care, aids residents in attending Life Skills and other enriching activities, fostering engagement.

Helps manage an inventory of activity and programming supplies, including games and crafts.

Schedules, communicates, facilitates, and documents monthly resident council meetings.

QUALIFICATIONS

Resident Engagement Coordinator

Education: An associate degree in Business Administration, Human Resources, Gerontology, or a related field focused on elderly services is preferred.

1 to 3 years of experience in senior living or skilled nursing activity and event planning, which may include moderate to large event planning responsibilities outside the senior living sector.

Demonstrated leadership skills are essential.

Knowledge and experience in the Assisted Living industry and Dementia care are preferred.

PHYSICAL DEMANDS
