Job Summary:

The Community Health Worker is a vital member of our team at Loma Linda University Medical Center, responsible for empowering patients and their families to navigate and access community services, healthcare services, and adopt healthy behaviors.


Key Responsibilities:

Supports providers and care management teams through an integrated approach to case management and community outreach, providing social support and informal counseling while advocating for individuals and community health needs.


Assists clinical personnel with community outreach, including home visits, health assessments, and involvement in clinical-community intervention activities.


Requirements:

High School Diploma or GED required; one year of experience working in a community-based setting preferred.


Experience working in a multi-cultural setting, knowledgeable about community resources, and able to provide referrals to community agencies as needed.


Knowledge of basic medical terminology, able to keyboard 35 wpm, and proficient in English and Spanish with professional quality.


Able to operate and troubleshoot basic office and patient care equipment, perform required patient care activities, and communicate effectively in English in person, in writing, and on the telephone.


Able to work calmly and respond courteously under pressure, collaborate and accept direction, and manage multiple assignments effectively.


Licensures and Certifications:

Valid Driver's License required at time of hire; successful completion of a Community Health Worker formal training program preferred.
