Job Title: Case Manager

PACE of Southwest Michigan is seeking a dedicated Case Manager to join our Social Work team. As a Case Manager, you will play a vital role in coordinating and overseeing the delivery of healthcare services to our elderly participants.

Key Responsibilities:
  • Participate in initial assessments, care planning, and periodic re-assessments of participants' cases.
  • Communicate participant updates to the Interdisciplinary Team and relevant parties.
  • Document updates in participants' medical records, including assessments and behavior modification plans.
  • Coordinate admissions and discharges from facilities deemed appropriate by the IDT.
  • Refer participants and families to community agencies or facilities, and act as a liaison with various organizations.
  • Participate in program and policy development, and actively contribute to the Quality Improvement Plan process.
  • Facilitate Medicaid application and redetermination processes with the Michigan Department of Health and Human Services.
  • Complete home visits as needed, and abide by all established PACE of Southwest Michigan policies, rules, and regulations.
Requirements:
  • Master's degree in Social Work.
  • Two years' experience in a health-related industry is preferred.
  • Licensed MSW in the State of Michigan, valid Driver's License with a vehicle and safe driving record, and CPR certification upon hire.
  • Ability to work a flexible schedule, and proficiency with technology, including the organization's electronic medical record system.
