As a Registered Nurse (RN) Case Manager, you'll play a critical role in delivering high-quality patient care in the comfort of their own homes. You'll be the point of contact for all disciplines involved in patient care, overseeing the frequency of visits and consulting with physicians and office staff as needed.

Responsibilities
  • Perform visits and coordinate client care
  • Oversee the frequency of visits for the episode
  • Consult with physicians and office staff as needed
  • Accurately collect OASIS data
  • Ensure medication profiles are up-to-date
  • Report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings
Qualifications
  1. Current licensure as a Registered Nurse (RN) in the state of employment
  2. A minimum of one year of clinical experience
  3. Demonstrate knowledge and skill in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
