At the April Parker Foundation, we are dedicated to making a meaningful impact in the lives of Transition Age Youth (TAY). Our APHomes Rapid Rehousing Program offers a unique opportunity for individuals to transition from transitional housing to permanent housing solutions.

As a Youth Transition Specialist, you will be part of a dynamic team that provides comprehensive support services to TAY participants. Your role will involve overseeing daily operations, responding to crises, and facilitating life skills training to ensure residents have the tools they need to succeed.

Program Objectives:

  1. Housing Stability: Provide safe, transitional housing to guide TAY participants toward permanent housing solutions.
  2. Supportive Services: Offer case management and tailored support focused on education, employment, and essential life skills.
  3. Community Integration: Create a strong sense of community, with the Coordinator offering guidance and fostering connections among residents.

Key Responsibilities:

  • Supervise and manage daily activities to maintain a secure and welcoming environment for residents.
  • Provide emotional support and guidance, helping residents navigate challenges and build resilience.
  • Respond to crises effectively, employing de-escalation techniques as necessary.
  • Assist residents in developing essential life skills, such as financial management, meal preparation, job searching, and time management.
  • Participate in program activities and events designed to promote personal and social development.
  • Maintain accurate records of incidents and daily occurrences for transparency and program evaluation.
  • Collaborate with case managers and staff to create personalized support plans tailored to each resident's needs.
  • Connect residents with resources for counseling, education, job training, and healthcare.
  • Promote a positive community atmosphere by ensuring residents understand and adhere to house rules.

Requirements:

  • High school diploma or equivalent; a degree in social work, psychology, or a related field is preferred.
  • Experience working with youth or in a residential setting is beneficial.
  • Strong communication and interpersonal abilities.
  • Effective problem-solving and crisis management skills.
  • Ability to collaborate and build relationships within a diverse community.
  • Patience, empathy, and a nonjudgmental approach.
  • Organizational skills and attention to detail.
  • Proficiency in Microsoft Office and experience with documentation systems.
  • First Aid and CPR certification (or willingness to obtain).

Salary: $21.62/hour
