Youth Support Specialist

We are seeking a highly skilled Case Manager II to join our team at the April Parker Foundation. As a Case Manager II, you will play a critical role in identifying and connecting individuals with accessible social support services. Your expertise will enable you to navigate complex systems and provide individualized support to clients. The ideal candidate will possess excellent communication skills, cultural competence, and a passion for advocating on behalf of vulnerable populations.
About the Job
In this role, you will have the opportunity to collaborate with community partners, assess client needs, and develop personalized plans to address their challenges. You will also be responsible for making timely referrals, ensuring continuity of care, and maintaining accurate records. This position requires strong analytical and problem-solving skills, as well as the ability to work effectively in fast-paced environments.
Duties and Responsibilities
- Conduct comprehensive client assessments to identify strengths, needs, and goals
- Develop and implement individualized service plans to promote client self-sufficiency
- Collaborate with community partners to ensure seamless transitions and continuity of care
- Make timely referrals to relevant resources and services
- Maintain accurate and up-to-date client records
- Participate in regular team meetings and training sessions to stay current on best practices and protocols
What We're Looking For
- Bachelor's degree in psychology, counseling, rehab counseling, or a closely related field
- A minimum of two years of related experience OR a Master's degree in social work, psychology, counseling, vocational rehabilitation, or a closely related field
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal skills
