About the Position

We are seeking an experienced Care Transition Specialist to join our team at SCAN Health Plan. In this critical role, you will be responsible for coordinating and managing the transition of members from the inpatient (acute/skilled nursing facility) setting to home or other care settings.

The ideal candidate will have a strong background in clinical nursing with a focus on transition of care activities from inpatient to home or other care settings. This role requires exceptional communication and interpersonal skills, as well as the ability to work independently in a remote setting.

About the Job

This is a 30-day program that starts at discharge, requiring the specialist to conduct comprehensive clinical assessments to develop personalized care plans and identify any medical, physical, and psychosocial needs to prevent utilization/readmission and support member's overall goals of care.

Key Responsibilities
  • Coordinate and manage the transition of members from the inpatient (acute/skilled nursing facility) setting to home or other care settings.
  • Conduct comprehensive clinical assessments to develop personalized care plans and identify any medical, physical, and psychosocial needs.
  • Ensure and evaluate clinical appropriateness of care plan by incorporating assessment findings against evidence-based guidelines, clinical reasoning, clinical practice guidelines and/or best practices in the community.
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree or equivalent experience.
  • A current & active California RN License.
  • 3+ years of related experience.
  • Technical expertise - Advanced analytical skills.
  • Problem Solving - Basic problem-solving skills.
  • Communication - Good communication and interpersonal skills.
