Lincolnshire Health and Rehabilitation Center - Position Overview:
The primary objective of this role is to deliver comprehensive nursing care and treatment to assigned residents under the supervision of the Director of Nursing.

All services rendered will adhere to established nursing standards, policies, procedures, and practices of this facility, as well as the regulatory requirements of the state.


Key Responsibilities:
While the following list outlines essential functions, it is not exhaustive and does not imply that these are the only tasks required of the position.
  1. Exhibit a thorough understanding and application of nursing policies and procedures in resident care administration.
  2. Oversee daily operations of nursing assistants in alignment with current regulations and guidelines governing the facility.
  3. Ensure compliance of all nursing staff with the facility's established policies and procedures.
  4. Provide residents and their families with exceptional service in a compassionate environment.
  5. Conduct meetings with assigned staff to coordinate shifts, services, programs, and activities.
  6. Adhere to facility protocols for documenting all activities during scheduled shifts.
  7. Assist in facility inspections conducted by authorized governmental bodies.
  8. Manage the admission, transfer, and discharge processes of residents in accordance with facility policy.
  9. Participate in all mandatory training sessions.
  10. Complete all necessary documentation related to resident admissions, transfers, and discharges.
  11. Accurately receive and transcribe telephone orders from physicians, recording them on the appropriate documentation.
  12. Document incidents, accidents, and changes in resident conditions promptly and accurately.
  13. Report any discrepancies regarding physician orders, dietary changes, or charting errors to the Director of Nursing or designated personnel.
  14. Maintain accurate documentation in residents' medical records, adhering to care standards and regulatory requirements.
  15. Review and finalize the 24 Hour Report as per policy.
  16. Ensure narcotic records are maintained according to standard nursing practices.
  17. Notify the Director of Nursing or designee of any discrepancies in drug and narcotic records observed during shifts.
  18. Properly manage all medications in compliance with applicable policies and procedures.
  19. Order, prepare, and administer medications and treatments as prescribed by physicians.
  20. Report any medication or treatment-related events to the Director of Nursing or designee.
  21. Develop work assignments and assist in completing necessary tasks as required.
  22. Foster positive working relationships with inter-departmental personnel and other facility departments to ensure that nursing and additional services meet residents' needs.
  23. Conduct nursing reports at the beginning and end of each shift.
  24. Report occupational exposures to blood, body fluids, infectious materials, and hazardous substances in accordance with facility policies.
  25. Perform nursing duties related to the pre-admission, admission, and discharge of residents.
  26. Conduct rounds throughout the shift to continuously monitor and assess residents' conditions.
  27. Accompany physicians during rounds as necessary.
  28. Consult with physicians regarding resident care and coordinate treatments as ordered.
  29. Conduct periodic checks to ensure prescribed treatments are administered and evaluate residents' physical and emotional well-being.
  30. Encourage and assist residents in participating in social and activity programs.
  31. Notify the resident's physician, family, and Director of Nursing or designee in the event of accidents, incidents, or changes in condition.
  32. Maintain cleanliness and orderliness of the nursing station, medication rooms, medication carts, and treatment areas.
  33. Administer standard nursing services, including catheterizations, tube feedings, suctioning, and dressing changes as required.
  34. Ensure resident care aligns with treatment plans and individual preferences.
  35. Respond promptly to all call lights.
  36. Monitor assigned personnel to ensure adherence to safety regulations in equipment and supply usage.
  37. Ensure all resident rooms are kept clean, safe, and sanitary.
  38. Comply with the facility's infection control protocols.
  39. Maintain adequate supplies of medications, medical supplies, and equipment to meet residents' needs.
  40. Participate in ongoing training and implementation of policies related to the safe operation of nursing service equipment.
  41. Ensure only trained and authorized personnel operate facility equipment.
  42. Review treatment plans to ensure appropriate and consistent care is provided.
  43. Adhere to facility policies regarding residents' rights.
  44. Maintain confidentiality of all resident information in compliance with HIPAA regulations.
  45. Report and investigate all allegations of resident abuse or misappropriation of property.
  46. Supervise daily activities of assigned nursing assistants.

Qualifications:
Education:
- Must possess a current Indiana LPN or RN license in good standing.
- Must be a licensed practical nurse or registered nurse.

Experience:
- No prior experience required; on-the-job training will be provided.

Specific Requirements:
- Ability to make independent decisions when necessary.
- Ability to interact tactfully with personnel, residents, families, visitors, and the public.
- Minimum age of 18 years.
- Must demonstrate patience, tact, a cheerful disposition, and enthusiasm while working with residents at various maturity levels.
- Willingness to explore new methods and principles and integrate them into existing nursing practices.

Physical Requirements:
- Ability to move throughout the workday.
- Must be able to communicate effectively in English.
- Ability to manage the mental and emotional demands of the position.
- Must possess functional sight and hearing, or utilize prosthetics to meet the position's requirements.
- Must function independently, exhibit flexibility, integrity, and effectively collaborate with patients, staff, and support agencies.
- Must maintain good general health and demonstrate emotional stability.
- Ability to relate to and work with ill, disabled, emotionally distressed, and occasionally hostile individuals.

Working Conditions:
- Work is conducted in various nursing service areas (e.g., resident rooms, shower rooms, nursing units, activity areas, dining areas).

- Movement throughout working hours is required.

- Subject to frequent interruptions and interactions with residents, family members, visitors, government personnel, coworkers, supervisors, and management.

- May require working beyond regular hours, including weekends and holidays, as necessary.

- Subject to call-back during emergencies such as severe weather or disasters.

- Participation in continuing education programs is expected.

- Exposure to infectious diseases and hazardous chemicals may occur.
