Job Title: Oncology Nurse Navigator

Job Summary:

The Oncology Nurse Navigator is a vital member of our multidisciplinary team, responsible for providing individualized assistance to patients, families, and caregivers to overcome healthcare system barriers and ensure timely access to quality care within a community oncology setting.

Key Responsibilities:

  • Coordinate care throughout the cancer continuum, identifying potential and real gaps in care delivery.
  • Prepare and lead Patient Care Conferences (huddles) at regular intervals, communicating interventional outcomes to the full care team.
  • Perform basic psychosocial assessments and provide emotional support, delegating and/or making referrals as needed.
  • Develop nursing diagnoses and corresponding action plans as needed.
  • Support and/or provide oral chemotherapy education, adherence, and side effect monitoring.
  • Participate in research, quality, and performance improvement, including reporting on navigation metrics.
  • Support patient advocacy and empowerment.
  • Participate in community outreach and cancer prevention education.

Qualifications:

  • Bachelor's prepared Registered Nurse (BSN), Oncology Certification (OCN) preferred, Nurse Navigator Certification a plus.
  • 5 (five) years work experience as a Registered Nurse in an oncology practice or clinic.

Critical Skills:

  • Demonstrated leadership, interpersonal, and collaboration skills.
  • Ability to apply the full nursing process in day-to-day clinical operations.
  • Ability to assess health disparities, identify needs of populations, and describe a patient navigation process to address those needs.
  • Ability to prioritize and re-prioritize quickly.
  • Strong organizational and critical thinking skills.

Work Environment:

The Oncology Nurse Navigator will work in a fast-paced, dynamic environment, collaborating with internal departments and external referral resources to provide high-quality care to patients and families.

Physical Demands:

The physical demands of this role include standing and walking for extensive periods, lifting and carrying items weighing up to 40 lbs, and requiring corrected vision and hearing to normal range.
