We are seeking a dedicated and compassionate Nurse Care Coordinator to join our team at Acute Home Healthcare. As a key member of our team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Develop and implement personalized care plans for patients
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare providers
  • Monitor patient progress and make necessary adjustments to care plans

Requirements
  • Current RN license in Florida
  • At least 2 years of experience in home healthcare
  • Valid Level II fingerprint screening
  • CPR/BLS certification
  • Updated TB/PPD test results
